横隔膜上食道憩室に対して腹臥位で胸腔鏡下食道憩室切除術を施行した1例

Thoracoscopic diverticulectomy for epiphrenic diverticulum in prone position：A case report

◆要旨：患者は50代，男性．食物残渣逆流を主訴に近医を受診した．精査にて横隔膜上食道憩室と診断され，手術目的に当院紹介となった．腹臥位で胸腔鏡下憩室切除術を安全に施行しえた．食道領域手術は時代とともに開胸手術から胸腔鏡下手術，縦隔鏡下手術へと変遷してきた．さらに体位では，従来の側臥位に対して，腹臥位の有効性が食道癌の領域で報告されるようになった．一方，食道癌以外の食道手術における腹臥位の報告はいまだ少なく，今回横隔膜上食道憩室に対して腹臥位で胸腔鏡下憩室切除術を施行した症例を経験したので報告する．

　A man in his 50s was admitted to a nearby hospital for regurgitation of food. An epiphrenic diverticulum was diagnosed and he was referred to our hospital for operation. Thoracoscopic diverticulectomy for epiphrenic diverticulum in the prone position was performed safely. Esophageal surgery has been transformed from thoracotomy to thoracoscopic surgery over time. Furthermore, the efficacy of the prone position for esophageal cancer has been widely recognized. However, reports of prone position in esophageal surgery other than esophageal cancer are rare. We reported a case of thoracoscopic diverticulectomy for epiphrenic diverticulum in the prone position.
