 For acute appendicitis during pregnancy, diagnostic method and surgical formula that take into consideration the effects on fetus should be selected. We herein describe laparoscopic appendectomy for a 35-year-old female at 27 weeks gestation who was diagnosed with appendicitis by MRI. An urgent laparoscopic appendectomy was performed. The uterus base had reached 3 cm above the navel ; however, careful port arrangement permitted safe completion of surgery without applying pressure to the uterus. Considering the fetal exposure and movement of the appendiceal position, MRI is a safe and useful diagnostic method. Furthermore, laparoscopic surgery may contribute to the continuation of a stable pregnancy of middle to late stages.
