腹腔鏡下幽門側胃切除後の体腔内吻合におけるステイプラー使用数節減のための工夫

Novel method for intracorporeal Billroth I reconstruction in totally laparoscopic distal gastrectomy

◆要旨：腹腔鏡下幽門側胃切除時のBillroth I法による体腔内吻合法として報告されているデルタ吻合やbook binding technique（BBT）は，リニアステイプラーを駆使した優れた再建法であるが，使用するステイプラーの本数が多く，日常臨床で汎用するうえで難点となっている．筆者らはBBTの再建法を応用して，ステイプラーによる十二指腸離断を省略しステイプラーの使用数を減らす工夫をしているため報告する．

As with development of laparoscopic surgery, totally laparoscopic distal gastrectomy, including several intracorporeal anastomotic techniques, has been introduced and its usefulness reported. The intracorporeal Billroth I gastroduodenostomy representative of Delta-shaped anastomosis technique and book binding technique is an excellent procedure, because it involves a wide lumen anastomosis. However, since a large number of linear stapler cartridges are required, some Japanese institutions are hesitant in making this a standard procedure. We propose a technique which could reduce the number of linear stapler cartridges in intra-corponeal Billroth I reconstruction after laparoscopic distal gastrectomy.
