ヘルニア中心小切開メッシュ吊り上げ法を用いた腹腔鏡下腹壁瘢痕ヘルニア修復術

A new technique for laparoscopic incisional hernia repair : A mesh lifting method from the center of the hernia sac

◆要旨：大きいサイズの腹腔鏡下腹壁瘢痕ヘルニア修復術において，メッシュ展開固定までの過程を容易にするため，ヘルニア中心部小切開を先行するハイブリッド法を考案した．ヘルニア中央に3cm程度の小切開を置いてはじめに開腹し，腹腔鏡を挿入して，側腹部にポートを留置する．その後外側のポートを用いて癒着を剝離し，中心部に糸を4本かけたメッシュを挿入する．それぞれの糸が頭尾左右に向くようにして中央小開腹創から体外に出し，密閉再気腹してメッシュ中央部を牽引固定する．左右のポートからメッシュ辺縁をtackingする．メッシュ中心を牽引固定しているため，辺縁の展開は腹腔内から容易であり，輪郭がたわむことなく，腹壁貫通支持糸なしでメッシュ固定を完了できる．

We developed a new method for performing a laparoscopic large incisional hernia repair which facilitates both the unfolding and tacking of a mesh fixation. A laparoscope is inserted from the center of the hernia sac through a small skin incision, and afterwards, ports are inserted into both sides of the lateral abdominal wall. A mesh with four threads is then inserted following adhesiolysis in the usual manner. The threads are thereafter pulled out extracorporeally from the center of the sac with an airtight cap, and then are placed on the cranial, caudal, right, and left sides, respectively. Because the center of the mesh is fixed tightly, transfascial suture fixation is unnecessary, and both the unfolding and tacking of the mesh can be easily performed.
