腹腔鏡補助下回盲部切除を施行した腸管Behçet病の3例

Laparoscopic assisted resection for Behçet's disease : report of three cases

◆要旨：Behçet病は難治性・再燃性の潰瘍性病変を呈する原因不明の炎症性疾患であり，経過中に消化管潰瘍病変を有する病態を腸管Behçet病と呼んでいる．近年，Behçet病やCrohn病のような再燃を繰り返す慢性炎症性疾患に対しての腹腔鏡下手術の報告が増えてきている．今回筆者らは腸管Behçet病の回盲部病変に対して，腹腔鏡補助下回盲部切除術を合併症なく施行できた3例を経験した．内科と手術のタイミングを図りながら，腸管の授動を十分に行えば，腹腔鏡下手術は慢性炎症性疾患に対しても安全に施行可能で有用な術式と考えられる．

Behçet's disease is an inflammatory disorder of unknown cause presenting refractory and recrudescent ulcerative lesions. The pathologic condition with gastrointestinal ulcerous lesions during the course is referred to as intestinal Behçet's disease. In recent years, there is an increasing number of reports of laparoscopic surgeries for chronic inflammatory disorders in which there is repeated recrudescence, such as Behçet's disease and Crohn's disease. The authors experienced three cases in which laparoscopic assisted lower ileocecal resection could be performed without complications for ileocecal lesions in intestinal Behçet's disease. In such cases, collaboration with the internist as to the timing of surgery and sufficient mobilizatoin of the intestinal tracts are important to laparoscopic surgery. Laparoscopic surgery is considered to be an operative procedure which can be safely performed and which is useful for chronic inflammatory disease.
