後腹膜巨大リンパ囊腫に対して腹腔鏡下囊腫壁部分切除による開窓手術を施行した1例

Laparoscopic marsupialization performed or a case of retroperitoneal lymphocelle

◆要旨：筆者らは腹腔鏡下開窓手術を施行した後腹膜リンパ囊腫症例を報告する．患者は38歳，女性．主訴は左腹部の腫大で，CT上160mm大の囊腫を呈した．エコー下に穿刺を行い，ピッグテイルカテーテルを挿入した．1,000ml以上のリンパ液の排液を認め,アルコールによる硬化療法を行ったが，穿刺後数週間で再度穿刺前と同様のサイズまで再発した．そこで筆者らはリンパ囊腫囊胞の前壁部分切除により大きな窓を作成することにより腹腔鏡下開窓手術を行った．後腹膜リンパ囊腫に対して腹腔鏡下壁部分切除による開窓手術は安全で侵襲の少ない治療方法であると考えられた．

　We report a case of retroperitoneal lymphocele opreatcd by laparoscopic marsupializaion. The patient was a 38-year-old Japanese woman having the chief compliant of left abdominal swelling. Computed tomography demonstrated a 160 mm cystic mass.

　We percutaneously punctured the lymphocele by using ultrasonography, inserted a pigtail catheter, drained over 1000 ml of lymphatic fluid and performed sclerotherapy with alchol. But, after few weeks the lymphocele returned to same size as before. Therefore we performed laparoscopic marsupializaion by creating a wide opening in the anterior wall of the lymphocele. Laparoscopic surgery for a retroperitoneal lymphoceles is a safe and minimally invasive procedure.
