Some methods of liver retraction to obtain a good view during laparoscopic surgery of the stomach have been reported. We have introduced a technique to uphold the liver using a Penrose drain and have modified it. Transfixing suture with 3-0 nylon is placed at each end of the Penrose drain(width 6 mm, length 12~16 cm). The center of the drain is sutured using 2-0 braid thread with needle at the tendon of the diaphragm above the esophageal hiatus. The right-side nylon is pulled up for lifting up the medial segment and the left-side nylon lifts up the lateral segment. Thus, the left lobe of the liver will be retracted without damage ensuring a sufficient operative field.
