Approaching the Bone and Soft Tissue using Subcutaneous Endoscopic Surgery

　皮下の比較的浅層に位置する骨・軟部組織へは，皮下内視鏡手術手技によりアプローチできる．まず，小皮切から直視下で小ポケットを作成し，先端に庇構造をもつ内視鏡で腔を確保しながら内視鏡下手術を行う．この方法は皮下浅層での手術であるため，直視下手術と内視鏡下手術の移行が容易であり，皮膚の伸展性を十分に利用して手術できることが多い．当然のことながら，腔を作成するための剥離層，手術対象とする組織の性状により手術時間と難易度は異なる．剥離操作が容易な骨膜下や筋膜上で腔を作成する場合は，手術操作を比較的迅速に行いうる．今後さらなる手術器具の開発と改良にともない，適応範囲は拡大されるものと期待される．

Subcutaneous endoscopic surgical techniques make it possible to approach the bone and soft tissue that are located in subcutaneous layer. First, subcutaneous dissection to create a small pocket is performed through a small access incision under direct visualization. Then, the dissection is continued to create on a optical cavity under endoscopic visualization using the endoscope equipped with a specially designed canulla. Selection of the dissection layer is very important to make the operation easy.
