子宮内膜症に対する腹腔鏡下手術

Laparoscopic surgery for endometriosis.

　1988年から1994年に160例の子宮内膜症患者に行われた腹腔鏡下手術の妊孕性に及ぼす効果を検討した．不妊症例における術後妊娠率は31.9％であり，術前ホルモン療法群（43.5％）は手術のみの群（25.4％）に比べて有意に高かった．Revised-AFS分類の進行期と術後妊娠率とは相関しなかったが，合計スコアが86点以上の例では術後妊娠率はきわめて低かった．また，付属器癒着の程度と術後妊娠率をみると，卵管癒着スコアが3点未満での妊娠率（46.7％）に比べて3点以上（15.2％）では低く，卵巣癒着スコアも10点未満（40％）と10点以上（13.0％）で差がみられた．これらの腹腔鏡下手術での成績は開腹手術における術後妊娠率とほぼ同様のものであった．しかし，腹腔鏡下手術では開腹手術に比べてより非侵襲的であるなどのメリットがあり，今後は腹腔鏡下手術が選択される症例が多くなると思われる．

One hundred sixty patients with endometriosis were evaluated after laparoscopic surgery performed between 1988 and 1994. The pregnancy rate was 31.9% in the previously infertile patients. Pre-surgical medical treatment for endometriosis significantly increased the pregnancy rate from 25.4% to 43.5%. The severity of endometriosis by the revised AFS score was not related to the pregnancy rate, but the pregnancy rate was very low in the patients with a total score over eighty six. The severity of tubal adhesion showed the highest relation to pregnancy outcome.
