Why is mottling skin “mottled”? —Mottling skin and livedo

mottling skin（mottled skin）は皮膚の循環障害によって出現する斑状の皮膚の色調変化であり，重症患者や敗血症性ショック時に微小循環障害を示唆する所見である。mottling scoreはmottlingが出現している範囲をスコア化したもので，ベッドサイドで微小循環障害を評価することができる非侵襲的で簡便なツールとして期待されている。敗血症性ショックの患者ではmottling scoreの変化から患者の予後を予測することができる。本稿ではmottling skinの病態生理を概説し，mottling scoreの臨床応用について解説する。

Mottling skin （mottled skin） is defined as mottled skin coloration and appears due to impaired circulation of the skin. Mottling skin is a finding suggestive of a microcirculatory disturbance in critically ill patients or in septic shock. The mottling score is a score of the extent of mottling. It is expected to be a simple, non-invasive tool for assessing microcirculatory disturbances at the bedside. In patients with septic shock, changes in the mottling score can be used to predict patient prognosis. In this article, we will outline the pathophysiology of mottling skin and discuss the clinical application of the mottling score.
