Cadmium accumulates in the kidney and damages proximal renal tubules. Urinary β2-microglobulin is not only measured to detect the early signs of toxicity, but is also used to judge severity and prognosis. However, since it can be affected by blood β2-microglobulin or unstabilized under a condition of low pH, it is recommended to measure simultaneously blood β2-microglobulin or other urinary indicators for tubular dysfunction. In a recent epidemiological study on inhabitants of formerly cadmium-polluted Jinzu River basin in Toyama, after restoration of rice paddies, overall affected renal function in them and patients with cadmium nephropathy at a risk of developing itai-itai disease were found by measuring urinary β2-microglobulin.
