白血球増多を示した耳下腺腫瘍—CSF産生腫瘍

Report of Undifferentiated Carcinoma of the Parotid Gland Associated with Leukocytosis: Colony Stimulating Factor (CSF) Producing Tumor

I．緒言

　最近悪性腫瘍患者でCSF（colony stimulating factor）産生腫瘍と呼ばれる成熟好中球の著明な増加を示す疾患がいくつか報告されている。一方耳下腺腫瘍の正確な術前診断は難しいものであり，いろいろな検査が組み合わされて行われているが，いまだに問題が残っている。今回われわれは耳下腺の未分化癌の症例で，CSFを産生し白血球増多を示したため，その診断に苦慮した症例を経験したので若干の文献的考察を加え報告する。

　A 52-year-old female with CSF producing tumor of the parotid gland was reported. Histological diagnosis of the tumor was undifferentiated carcinoma. The patient revealed remarkable leukocytosis, the highest 84,900/mm³ with 92% neutrophils. No blast forms were seen in the blood smears, and the results of sternal puncture indicated no evidence of leukemia. CSF producing tumor was suspected, and a higher level of CSF was detected in the conditioned medium of the tumor cells. Radiation and anticancer drugs were administered, but the tumor was gradually growing, and the patient died,
