Lateral Rhinotomy: The Surgical Technique and Indications

I．はじめに

　1902年にフランスのMoure^1）が外切開法による篩骨洞へのアプローチを報告して以来，欧米においてはこの手術テクニックに種々の工夫が加えられ，lateral rhinotomyとして現在広く用いられている^2〜5）。この手術術式を用いると，篩骨洞をen blocとして切除し，上顎洞の内側半分を切除することができる。しかし本邦においては，残念ながらこの手術方法に関する報告は少なく，またわが国において出版されている耳鼻咽喉科教科書，手術君等にもその手術術式の詳細な記載は見られない。我々は鼻腔腫瘍および腫瘍の本体が鼻腔側壁方向に偏位しているような上顎洞腫瘍，それから鼻腔，上顎洞より篩骨洞へ進展している腫瘍に対して，この手術術式を用いてきた。その結果，腫瘍をen blocに切除し，満足しうる治療成績を得ているので，その手術手技の詳細について紹介したいと思う。

　Lateral rhinotomy offers a surgeon a wide visual access for en bloc removal of tumors in the ethmoid sinus and nasal lateral wall. This surgical technique can be available for the removal of malignant tumors as well as benign ones. Ten patients who had malignant or benign tumors in the nose and paranasal sinuses underwent en bloc ethmoidectomy and medial maxillectomy by this technique, and the favorable results were obtained. The details of this tecnique and the indication were discussed.
