挿管麻酔後の喉頭合併症—1．喉頭肉芽腫

Complication of Tracheal Intubation : Laryngeal Granuloma

　I．はじめに

　全身麻酔や緊急気道確保を目的とした気管内挿管は，最近では日常茶飯事となっているが，この多くの気管内挿管症例のうち，低頻度ではあるが，挿管の影響を受けて生じる合併症をみることがある。このような合併症にも，2〜3日で気付かれないうちに治癒してしまうものから，2年以上経過しても治癒しない神経麻痺，治療の効なく再三，再四，再発する喉頭肉芽腫，喉頭横隔膜症，声帯癒着症など，その種類，程度は多種多様である。

　こうした気管内挿管後に起こる合併症を，Applcbaumら^1）の"Tracheal Intubation"より引用すると，表1のごとくであり，筆者らの経験から，比較的稀に認められる疾患に星印を付した。今回は，このうち京都府立医科大学耳鼻咽喉科学教室喉頭外来で，過去4年間に診断，治療を行った7例の喉頭肉芽腫について検討した。

　Laryngeal granuloma, except for contact granuloma, is a late complication of tracheal intubation. The incidence was reported to be 1:10000 to 1:20000 by Snow et al., and the usual location is in bilateral vocal process of the arytenoid cartilages. Seven cases of laryngeal granuloma diagnosed and treated in the laryngeal clinic of Kyoto Prefectural University of Medicine, are reported. Respiratory movement of the vocal cords while a tube was in place and vocal abuse after extubation were thought to be predisposing factors of laryngeal granuloma.
