A case of spontaneous cerebrospinal otorrhea due to congenital abnormalities of the stapesand the otic capsule was presented.

This 7-year-old boy developed recurrent attacks of bacterial meningitis and he was proved as having profuse cerebrospinal fluid otorrhea at myringotomy. X-ray examination of the temporal bone demonstrated abnormal bony labyrinth. At operation, a fistula was demonstrated in the footplate of the stapes through which the cerebrospinal fluid was flowing into the middle ear. Fascial plugging of the vestibule resulted in a satisfactory repair of the fistula and prevented subsequent spread of infection to the meninges.
