30年後に癌化した小児喉頭乳頭腫症例

INFANTILLE LARYNGEAL PAPILLOMA THAT TRANSFORMED INTO A CARCINOMA AFTER THIRTY YEARS INTERVAL

　I．緒言

　小児喉頭乳頭腫は小児喉頭腫瘍のうちでもつとも頻度が高く，また再発性に富み，難治である。しかし組織学的には良性腫瘍の形態を示し，前癌状態といわれている成人の乳頭腫とは異なつて悪性化はきわめて稀である。

　われわれは乳幼児期に再発を繰返し，きわめて難治であつたが最終的にはRa照射によつて治癒した喉頭乳頭腫が，約30年後に悪性化を呈した症例を経験したので，臨床経過，病理組織学的知見ならびに治療について報告し，併せて若干の考察を行なう。

　Malignant changes of laryngeal papilloma during childhood is exceedingly rare.

　The patient was a 34-year-old woman who suffered laryngeal papilloma during infancy.

　The papilloma had been treated by repeated surgical removal and radium irradiation for the period of 4 years which was finally controled. However, after the interval of 30 years thereafter, the patient complained of severe dyspnea. Biopsy taken from the laryngeal cavity showed squamous cell carcinoma. 　It indicates in this case that the laryngeal papilloma had transformed into malignant tumor and the radium irradiations had accerelated that tendency.
