縦隔鏡検査の一経験—稀有な縦隔腫瘍の1例

MEDIASTINOSCOPY IN THE DIAGNOSIS OF MEDIASTINAL TUMORS, A CASE REPORT

　I．はじめに

　胸骨上窩に皮膚切開を加え，そこから内視鏡を挿入して上，中，後縦隔内を直接観察し，あるいは触診，生検などを行なう検査法は，1959年Carlens^1）によつて考案され，mediastinoscopy縦隔鏡検査法と命名されたが，この方法はその後，Palva^2），Massen^3），Link^4），Reynders^5）その他多くの学者により追試され，各種肺・縦隔疾患の診断上その価値が広く認められて今日に至つていることは周知の通りである。

　縦隔鏡検査法は，当初より，主として胸部外科領域に導入，活用されているが，最近では，気管支鏡や食道鏡と並んで，耳鼻咽喉科領域における内視鏡手技の1つとして内科，外科などからの依頼も増加する傾向にあるので，同手技の実際や，適応，禁忌などについてその基本事項をまとめ，併せて，最近われわれの教室において経験した稀有な縦隔腫瘍の一症例につき報告する。

　Since mediastinoscopy has been introduced by E. Carlens in 1959, a significant diagnostic advancement has been made in the evaluation of in trathoracic diseases.

　The mediastinoscopy in the past has been chiefly resorted to by chest surgeons. Today however, it is employed by internists and otolaryngologists as well, along with bronchoscopy and esophagoscopy.

　The authors note the crucial points of indications, surgical technique, and the limitation of its application.

　A case of mediastinal tumors, revealed by this method, containing paratrachial lymph node cancer metastasized from the thyroid glands, a large chondromyxosarcoma with six small lipomas, is reported.
