口蓋裂手術の反省

A RETROSPECTION ON STAPHYLORRHAPHY

　Ⅰ．緒言

　口蓋裂の形成手術は，破裂部が完全に閉鎖し，言語機能が正常に復して初めて成功したといえる^1）2）。ために，Ernst，Dorrance，Veau，Wardill，永井などが言語機能の回復に重点をおいて，V. Langenbeckの原法に種々の改良を試みてきた（藤野の口蓋裂治療法の発展^4）による）。しかしながら，症例によつては，破裂の閉鎖すら困難を感ずるものが少なくない。我々の教室で行なつた口蓋裂手術患者のうち，口蓋裂閉鎖に苦心した症例3例をのべ，今後の症例への参考に供したいと思う。

　Staphylorrhaphy was performed in 3 cases respectively aged 4, 23 and 32 years; the operations were failure in all 3 cases. The principle cause of failure in retrospect appeared to be as follows:

　1) Insufficient care in disposal of tissues surrounding the greater palatine artery. 2) Insufficiency in the separation of the periostium at the posterior edge of the bony palate. 3) Inadequacy in suturing of the muscular layers.
