本例は言葉が通ぜず,家人はおらず,呼吸困難甚しく氣管及び食道の直達検査も困難,レ線検査も不可能で,然も突然死亡し死因究明にも迷つたが幸い死因調査で剖検に附せられたので種々の徴候に對する説明も出來た,CPCに好適例である。
患者:28年の鮮人女,日雇勞働者の妻,本年4月23日朝7時半頃蓬餠の煮たのを汁は飲まず急いで食べたところ食道入口部に停滞したらしく汁も水も通らなくなり,且甚しい呼吸困難を生じたので同居人が附近の醫師に運び續いてパトロール車に依頼8時10分來院した。
YAMAKAWA and OHKOCHI report an interesting case of mediastinal cancer tnat re-sulted in sudden death of the patient from asphyxia. A woman aged 28 complained of acute dyspnea after eating 'mochi' which is highly resilient pounded rice cake, and, which apparently lodged firmly in the esophagus. Be-cause of increased dyspnea by placing the patient in dorsal recumbent position emergency tracheotomy was performed with the patient sitting upright. Esophagoscopy was followed but, because of increased dyspnea the procedure had to be intercepted before removal of the offending foreign body had been accomplish-ed. Relief from dyspnea was attained only after insertion of spiral tracheal canula. And in the meantime the esophageal obstruction passed on into the stomach; the patient was able imbibo water without any difficulty. The spiral canula was, now, interchanged with the ordinary type, but, 20 minute later the patient was dead from asphyxia. Necropsy revealed mediastinal gland cancer grown to a size mea-suring 11X10.5 X 3cm and, found to be press-ing on the trachea.
