緒言
古來副鼻腔内炎症を確実に診断する爲に,副鼻腔内造影剤注入に依るレ線檢査が利用されている.
吾國では久保(猪)教授,兒玉氏を始め多数の先人が,副鼻腔の一つである上顎洞内に造影剤を注入して洞の大小形状,粘膜の肥厚度等の形態像をレ線的に観察している.又King1),Müller2),最近ではProetz3)は上顎洞に注入された造影剤の排泄状況を観察し,洞内粘膜の機能像即ち氈毛運動に就て考察している.
In view of investigating the function of elimination in the mucous membrane of the maxillary sinus; Daito and Ishikawa employed a 40 per cent solution of molyiocdol to be instilled into the sinus. This was studied in vivo by series of radiograms and later by microscopic sections of the mucous membrane. In order to avoid possible influence of nasal respiration on this elimination the subject of the study was selected in a patient who had been previously laryngectomized.
In the first 15 days of observation grandual elimination of the radioopaque body was shown by casting of partial and incomplete shadow. By the 20th day, this shadow was localized to an area and the size of it decreased to that of the tip of little finger. The process of elimination, then on, was exceedingly slow in accomplishment and, it was by the 49th day before it had completely disappeared.
At necropsy, it was shown that the area, that which corresponded by situation and size to the final localization of the opacity, was located by accompaniment of hypertrophic mucous membrane on the posterior wall of the sinus opposite the greater palatine sulcus. The mucous membrane of the rest of the sinus was found to be normal, the both of which were confirmed by microscopic studies.
The authors conclude from these findings that, there is a tendency in the normal mucous membrane to eliminate the object of elimination by a gradual process which will be shown by casting of partial and incomplete shadows if the material instilled is radioopaque ; elimination by the diseased membrane, on the other hand, would be delayed and difficult, thereby casting shadows which would be solid and complete in contrast to that of the normal area.
