Ⅰ．はじめに

　原発性meningeal melanocytomaは，稀な疾患であり，頭蓋内では後頭蓋窩やMeckel's caveに多いとされ，比較的予後良好といわれている^5,20）．本症例は中頭蓋窩底部から左側頭葉に進展した原発性meningeal melanocytomaであり，小脳テント上では21例目の報告である．Meningeal melanocy-tomaはmalignant melanoma, pigmented men-ingioma等との鑑別が問題となる．しかし，その鑑別は画像上では困難な場合が多い．今回われわれは，術中にこのような症例に遭遇した場合どう対応すべきか，外科的な立場から考察したので報告する．

A 29-year-old woman presented with a primary meningeal melanocytoma of the left middle fossa manifesting as headache and nausea. Computed tomography (CT) with contrast medium demonstrated a clearly demarcated, homo-geneously enhanced high density area surrounded by a cyst in the left temporal lobe. Magnetic resonance (MR) imag-ing showed the mass as a slightly high signal intensity area on the T1-weighted image and as a low signal intensity area on the T2-weighted image. Cerebral angiography revealed shift of the middle cerebral artery but no tumor stain.
