Ⅰ．はじめに

　慢性硬膜下血腫は血腫の除去のみで治癒にいたるため，最近では穿頭による血腫洗浄術が一般的となっている^2）．しかし，手術方法に関係なく^3,10）10％近くの再発がみられ^4,9,14），これまで病態に応じた再発予防が講じられてきた^11,16）．再発要因は様々なものが指摘されているが^5,8,13），手術によって対策が可能な要因として，術後の残存空気が挙げられる^1,6,7）．われわれは，内視鏡を用いて血腫腔内を観察し，その所見を参考にして術後残存空気の速やかな排出を目的とするドレナージの血腫最前挿入を試みてきた^12）．今回，ドレナージの挿入方向と術後残存空気，および再発率との関係について解析検討し，二，三の知見を得たので報告する．

The favorable effect of burr hole irrigation and closed drainage system has been reported in the treatment of chronic subdural hematoma (CSH). After analyzing the relationship of the direction of the drainage tube and recur-rence12), we have suggested that the residual all after surgery night he important as one of the factors causing recurrence. The present study included 128 chronic subdural hematomas in 102 patients who had been treated in our institute between January, 1996 and October, 2000.
