Ruptured peripheral lenticurostriate artery aneurysm in a child:case report

I．はじめに

　レンズ核線条体動脈末梢の動脈瘤の多くは高血圧性脳内出血の出血源である微小動脈瘤であり^26），通常の血管撮影で発見される例は，もやもや病，AVMなどの脳血管奇形を合併していることが多い．今回，高血圧や脳血管病変を合併しない女児で，脳内出血で発症し，レンズ核線条体動脈最外側枝末梢の動脈瘤が出血源であった1例を経験したので報告する．

We report a 12-year-old girl with intracerebral hema-toma and subarachnoid hemorrhage secondary to the rupture of a peripheral lenticulostriate artery aneurysm. We carried out proximal clipping of the lateral lenticu-lostriate artery at the middle cerebral artery following six days of continuous ventricular drainage for acute hydrocephalus. Postoperative angiography confirmed the obliteration of the aneurysm. The girl was dis-charged without any neurologic deficits. To our know-ledge, this is a first reported case in the literature of a ruptured peripheral lenticulostriate artery aneurysm in childhood. The pathogenesis of such an aneurysm and the treatment modalities are discussed.
