I.はじめに
median artery of corpus callosum(以下MACC)は前交通動脈の一分枝であるが,これが太くなり前大脳動脈(以下ACA)の支配領域を養うようになった場合はaccessory ACAとよばれ,血管奇形とみなされる.最近われわれは,脳血管写上MACC(accessory ACA)を認め,同部に発生した破裂脳動脈瘤の1手術症例を経験した.われわれの渉猟しえた限りでは,MACCに動脈瘤を認めた症例の報告はなく,極めて稀と思われたので,若干の考察を加え報告する.
A rare case of ruptured cerebral aneurysm of median artery of corpus callosum (accessory anterior cerebral artery: Acc ACA) is reported.
A 66-year-old hypertensive female suddenly lost consciousness for 5 minutes and later complained of severe headaches. On admission, the next day, consciousness was clear and she showed no neurological deficit except for right motor weakness. CT scan revealed subarachnoid hemorrhage, and carotid angiography showed triplicated anterior cerebral artery with a saccular aneurysm on the Acc ACA. Twenty hours after the onset, an operation was performed to clip the aneurysm neck.
