Ⅰ．はじめに

　神経放射線学，神経耳科学，Microneurosurgeryの進歩によって聴神経鞘腫の早期診断，早期手術が行われうるようになったが，必ずしも患者が早期に受診するとは限らず，かなり大きな腫瘍となってはじめてわれわれの所へ送られて来る場合も少なからずあるというのが現状である．

　聴神経鞘腫の診断における神経放射線学の果たすべき役割は，腫瘍が非常に小さい場合にはその存在を確認することであり，臨床的に腫瘍の存在が明らかな場合には周囲脳組織との関係，腫瘍への血液供給の関係を明らかにし，手術操作の決定に寄与することであると考えられる．前者の目的で内耳道撮影およびcerebello-pontine cistern,internal auditory canalのpositive meato-cisternographyが行われ，後者の目的のためにvertebralangiographyが行われることになる．ゆえにcontrast studyの適応としては，臨床的に第8神経のみが侵されていればpositive meato-cisterno-graphyがfirst choiceであり，第8神経以外の脳神経が侵されていたらvertebral angiographyがfirst choiceで，それで確診がつかなければpositive meato-cisternographyを行うべきであると思われる．
