実験的中大脳動脈閉塞症に対するalphamethyltyrosineの効果

Ⅰ．はじめに

　近年，頭蓋外脳動脈と共に頭蓋内脳主幹動脈閉塞症に対しても積極的に血流再建術が施行されるようになったが，血流再開後かえって出血性梗塞をきたして症状を増悪させる症例も知られ，あらためて手術適応について検討がせまられている．

　1967年Thompsonら^16）は頸動脈の完全閉塞症100例に対するendarterectomyの結果を報告し，6時間以内に血流再建術をおこなった4例の全てに良好な結果を得，6時間から24時間以内に手術を施行した場合の成功率は63％であったと述べ，最も重要な因子は血管閉塞から手術（血流再開）までの時間であることを指摘し，早期手術の重要性を強調した．Crowellら^1）は動物実験で脳血管の閉塞時間に比例して脳虚血性変化が進行することを示し，4時間以内に血流を再開させれば出血性梗塞の危険はさけられるのではないかと述べた．

A double blind prospective study of the modification by alphamethyltyrosine (AMT) of experimental occlusion of the middle cerebral artery (MCA) was performed on squirrel monkeys. The transorbital microsurgical technique was employed to expose the origin of the left MCA.

A miniture Levis-Scoville clip was placed on the left MCA for 3 hours.

Thirteen monkeys were given 10 ml/kg of 2% AMT solution approximately one hour prior to occlusion and just after revascularization. Thirteen control monkeys were given an equal volume of normal saline.
