術前の出血に対して栄養血管の超選択的塞栓術が有効であった頭皮部angiosarcomaの1例

Superselective Feeder Embolization for Hemorrhage from Cutaneous Angiosarcoma in Scalp

Ⅰ．はじめに

　Angiosarcomaは，血管などの内皮細胞に由来する悪性間葉系腫瘍であり，好発部位として皮膚，軟部組織，肝臓などが挙げられる．特に皮膚では高齢者の頭部，顔面部に発生し予後は極めて不良である．発生頻度が稀で，しかも治療の主体は皮膚科が担うことが多いので，脳神経外科医には馴染みが薄いものであるが，頭部打撲や外傷が誘因となることもあり脳神経外科領域においても鑑別診断，早期診断の知識が必要と考えられる．また，結節を形成したものは出血を呈することがあり，全身状態の悪化を招く．今回われわれは，頭部打撲を誘因として発症した頭皮部の結節状angiosarcomaが経過中に腫瘍表層から出血を来したため，術前に腫瘍栄養血管塞栓術を施行し，止血を得た症例を経験したので若干の文献的考察を加え報告する．

A 76-year-old male developed a hemorrhagic nodule and a surrounding purple-red,irregularly shaped macule on his scalp after head trauma. The diagnosis of angiosarcoma was confirmed histologically. A transarterial feeder embolization was performed followed by a surgical excision. It was effective for bleeding from a tumor,and superselective catheterization was important for this specific tumor. The early diagnosis by neurosurgeon may lead to improved survival.
