半側空間失認

Unilateral spatial agnosia (neglect)

はじめに

　半側空間失認unilateral spatial agnosia，または半側空間無視unilateral spatial neglectは，外空間の半側にある対象を無視する症状^41）であり，行為の障害と不可分に結びついた失行—失認症apractognosiaとしてとらえられており^2,47），象徴機能の障害の意味での失認agnosiaとは区別される^39）。

　日常生活の中で，患者は常に一側（病巣側）に視線を向けている，歩行時に一側（病巣側）に片寄る，病巣の反対側にある障害物に衝突する，病巣と反対側への曲がり道を無視して迷子になる，食卓で半側（病巣の反対側）を見落とすなど，視覚的な半側空間の無視としてとらえられることが多く，半側視空間失認unilateral visualspatial agnosiaとして論じられることが多かったが，触覚空間^10），聴覚空間^23）など，視覚以外の感覚様態にても半側の無視は出現し，この点でも従来の失認の概念からはみ出している^45）。

　The term "unilateral spatial agnosia (neglect)" refers to the syndrome neglecting objects in one half of the extrapersonal space (usually the left). As the syndrome unseparably related to the disturbances of praxis, it has often been termed "apractognosia", and separated from pure "agnosias" for the sense of disturbances of symbolic function.

　Since Brain (1941) related this syndrome to the right hemispheric lesions, most authors have stated the predominance of right hemispheric lesions.
