I．はじめに

　一般にSEPは，たとえば正中神経を皮膚上から電気的に刺激して誘発される。これら末梢神経は混合神経であるから，もちろん運動神経も含まれるが，SEPが頭皮上から誘導されることは，求心性知覚線維が刺激されたことによる。求心性知覚線維には，深部感覚また皮膚感覚に関する知覚線維，さらに皮膚感覚には原感的（protopathic）な触・圧感覚と痛覚・温度覚や識別的（epicritic）な触・圧感覚に関する知覚線維が含まれる。末梢神経を皮膚上から電気的に刺激して，対側体知覚領相当部頭皮上から得られるSEPは，これらすべての求心線維が刺激されて生ずるものなのであろうか。

　神経刺激によるSEPを生ずる上行性伝導路に関しては，及川（1978）が総説したように，SEPの早期成分および後期成分の一部がともに後索路，内側毛帯，視床VPL核の系に由来するという説の方が強い。これはDesmedt（1971）やHalliday（1975）の感覚喪失患者やVPL核実験障害例などの結果からの総説的結論である。であるとすると，外側あるいは前脊髄視床路を経由するインパルスは，電気刺激によるSEPには寄与しないのか，という疑問が生ずる。一般には，痛覚は外側脊髄視床路によると考えられている（たとえば草間，1967や草間ら，1977）。
