I．はじめに

　Herpes simplex encephalitis（H．S．E．）は散発性の激症脳炎であり，その現象形態からみると新生児に全身感染症の播種性病巣として生ずる型と成人にみられる大脳辺縁系を主として侵す急性壊死性脳炎型とがある。近年herpes simplex virus（H．S．V．）の抗体価の上昇および剖検で壊死性悩炎がみられる例をH．S．E．として報告されているのがしばしばみられる。しかし剖検時に脳実質よりH．S．V．の分離，あるいは螢光抗体法にて脳実質内にH．S．V．の抗原を証明して，その診断の確定している報告例は，本邦では1974年までに4例しかなく成人例でウイルスの型分類まで行ないえた症例はいまだみられない。最近われわれは13日間の臨床経過で死亡し，剖検時脳組織内に螢光抗体法にてH．S．V．抗原を証明し，しかもH．S．V．の分離培養にてその型分類に成功し，H．S．V．type 1と決定した症例を経験したので報告する。

　A 34 year-old male admitted to the hospital with the chief complaints of incoherent speech and tremor of the right arm on April 16. Family history and past history were unremarkable. On April 12 he began to complain of ear ache and a few days later he noticed high fever up to 38°C and began to respond poorly for questions and behaved abnormally.
