判別関数を利用した老年者の脳血管発作の鑑別診断

Differential diagnosis of cerebrovascular stroke of the aged: Analysis of 176 cases with linear discriminant function

I．はじめに

　わが国の脳卒中による死亡者は昭和47年には約17万6千人を数え，ここ数年来増加の傾向こそ見られないが依然として死因の第1位を占め続けており^1），われわれの最も身近な致死的疾患の一つということができよう。脳卒中という術語は，がんらいは"脳の循環障害によって起こる急激な意識障害と運動麻痺を合併した臨床症候群"に対して定義されたものであろうが，片麻痺や言語障害などの中枢神経症候が比較的急速に発現してきているにもかかわらず，意識障害を伴わなかったり，軽度であったりする症例もしばしば観察されるので，最近ではそのような場合も含めて使われることが少なくないように思われる。

　私たちもこの小論で，後者の意味での脳卒中症例を対象として調査したが，本来の定義と区別するために脳血管発作という術語を使用することにした。

By means of the combination of Fisher's linear discriminant function and branching logic, differentiation of the cerebrovascular stroke was made with regard to localization and pathological nature of the lesions. Although there were many causes of the stroke, we devoted ourselves to the differentiation of cerebral hemorrhage, cerebral infarct and subarachnoid hemorrhage of the aged.
