Vertical gaze palsy as difined by Parinaud accompanied paralysis of convergence which, however, does not constitute an essential part of the syndrome.

Isolated upward gaze palsy is caused by bilateral lesions of the posterior commissure together withits interstitial nuclei.

Downward gaze palsy is produced by lesions of the bilateral oral Edinger-Westphal nuclei and retroflexed fasciculi as they pass dorso-medial to the red nuclei.
