核間性眼筋麻痺

Internuclear ophthalmoplegia

I．はじめに

　側方視に際して内転眼の内転障害と外転眼の外転側に向かう単眼性眼振があり，それでいて輻輳は可能な現象は，1902年にBielschowsky^1）が初めて記載した。その後同様の現象を示す症例が表1のように次々と報告され，表2のように種々の名称で呼ばれた。中でも，1921年Lhermitte^4）は，嗜眠性脳炎の病態生理で本現象に注目し，これが眼球運動核間の連絡線維束の障害によるとする考え方を初めて導入し，"paralysie internucléaire"と呼んだ。これが今日用いられている核間性眼筋麻痺の語源である。1923年Lutz^5）は，両眼の内転麻痺があるが輻輳可能な21歳男子の軽症脳炎と考えられる1臨床例を報告し，"ophthalmoplegia internuclearis anterior"と呼んだ。これに対して，実際の症例はないが側方視に際して外転のみ麻痺がある可能性を想定して，これを"ophthalmoplegia internuclearis posterior"とした（図1）。

Since the first report by Bielschowsky in 1902, there have been many case reports of internuclear ophthalmoplegia (INO). Those reported cases of INO were reviewed with particular importance placed on the evolution of its terminology and concepts. Various tests and their characteristics for documenting INO were described. Underlying diseases of INO were discussed according to the literature as well as to the author's cases.
