MLF症候群における垂直眼球運動障害

The disturbance of vertical eye movement in MLF syndrome

緒　言

　MLF症候群^1〜3）は核間麻痺inter nuclear oph-thalmoplegiaと言われているもので，内側縦束medial longitudinal fasciculus （以下MLFと略す）の障害により発生するとされている。MLFは，中脳上端より延髄下方に及ぶ線維で前庭神経核，眼球運動核を連絡している。一側のMLF症候群の症状は，（1）患眼の内転障害，（2）反対側眼の外転時に発生する単眼性水平性眼振，（3）輻輳運動は正常であるという，三つの症状とされている。以上の様に，MLF症候群は，水平性眼球運動障害として研究され，分類された。Cogan^4）はMLF症候群を3型に分け，内転障害に輻輳障害も加わつているものを，前部型（anterior type）としMLFと外転神経核の障害，または，MLFと橋傍内側網様体　（Parapontine Reticular For-mation，以下PPRFと略す）の障害のあるものを，後部型（posterior type）とし，他のものを，中間型（midzone type）と分けている。posteriortypeで，MLFとPPRFの障害では，患側眼の内転，外転障害と，健側眼の内転障害も起こることより，one and half syndrome^5，6）とも紹介されている。しかし，MLF症候群における垂直眼球運動についての研究は少ない。

A 54-year-old male developed acute unilateral medial longitudinal fasciculus (MLF) syndrome with dissociation of vertical oculomotor distur-bance. Electrooculography (EOG) showed hori-zontal saccades indicating the presence of ipsila-teral abducting palsy. The condition was diag-nosed as Cogan's posterior type of MLF synd-rome.

Vertical EOG showed the vertical saccadic movement to be normal but the ocular fixation at eccentric positions could not be maintained.The vertical smooth pursuit movement, on the other hand, was disintegrated into smoll soccades.
