小児医療的側面—とくに重症心身障害児（者）200例の5年間のFollow-upの結果から

Medical treatment of the severely mentally and physically handicapped children and the results of the five-years-follow-up study

　重症心身障害児は行政的には「重度の精神薄弱および重度の肢体不自由が重複している児童」と規定されている。また18歳以上の者については児童福祉法63条で，児童と同じ施設に収容してよいことになっている。しかし医学的観点からみるとき，「重症の脳の発達障害」^1）として把握しうる。かかる重症の慢性神経疾患に対する対策としては医学の立場からは単に小児科だけではなく，神経科，整形外科，精神科，眼科，耳鼻科，歯科といった総合的なアプローチが必要である。また麻酔科，脳神経外科，一般外科，婦人科，泌尿器科などの外科的治療も場合によっては必要となる。しかし現実の重症心身障害児施設は単独の施設であったり，結核療養所（一部精神療養所）に併設されたものがほとんど大部分である。ごく一部に小児病院や総合病院に併設されたものがあるにすぎない。都立府中療育センターは昭和43年4月に開設されたが，できるだけ総合診療的アプローチを行なえるよう計画され，小児科，精神科，整形外科，一般外科，脳神経外科，検査科の常勤医が勤務し，眼科，耳鼻科，歯科は非常勤の形態で診療に従事している。

Two hundred cases of the severely handicapped children (physically as well as mentally retarded) were studied for a 5 years period. The results of the study mainly concerned with an aspect of pediatric medicine were discussed. Of 200 cases, 33 died, 17 discharged or transferred to other institutions. The remaining 150 cases were studied in detail.

1. Concerning motor disturbances, improvement was observed in 23.3% of the cases, 72.0% showed no changes, and 4.7% regressed. Concerning mental disturbances, 8.7% were improved, 90.6% not changed, and 0.7% regressed.
