Polyneuritis Cranialis経過後2年にしてMyasthenia Gravisの発症した1症例

Polyneuritis Cranialis and Myasthenia Gravis occurred with an Interval of 2 Years

　症例は25歳，女性。42年7月雨に濡れたあと鼻声，嚥下障害が出現し，8月当科外来を受診，眼輪筋麻痺，兎眼，軟口蓋挙上不全，顔面筋麻痺が出現していた。42年9月当科入院所見では，眼瞼下垂（－），眼球運動正常，眼輪筋力弱，兎眼（＋），口輪筋力正常，軟口蓋挙上正常，鼻声（＋），四肢筋力正常，筋萎縮（－），知覚正常，深部反射上肢正常，下肢亢進，髄液検査正常，Tensilon test（－），筋電図では眼輪筋にて放電数減少，右尺骨神経最大伝導速度62.1m/sec。眼輪筋麻痺，兎眼，と軽度の鼻声を残すのみで外来受診時よりやや軽快していた。この時行なつたTensilon testでは眼輪筋麻痺，鼻声に効果が見られなかつたためpolyneuritis cranialisと診断され，steroid，B_1）投与を行ない，42年11月には軽快退院した。その後順調であつたが，43年6月には鼻声が出没していた。44年12月には再び易疲労，兎眼，鼻声，嚥下咀嚼障害が現われた。これらの症状は45年3月に感冒罹患後増悪，45年4月再入院した。

A 25 year old woman was healthy untill July, 1967 when she noticed nasal voice, weakness of facial muscles and disturbance of swallowing after getting wet with rain. Examination revealed lagophthalmus, weakness of orbicular oculi and oris muscles, nasal voice and limited elevation of soft palate. On September 12, at the time of admisson, symptoms and signs were generally improved except for slight lagophthalmus and slight nasal voice.
