老人性組織病変，小動脈硬化を伴う亜急性海綿状脳萎縮症（Spongiform Cerebral Atrophy）の1剖検例

An Autopsy Case of Subacute Spongiform Cerebral Atrophy with Senile and Vascular Lesions

I．はじめに

　初老期に発病する精神病（原発痴呆）の疾患群のなかには老年痴呆，Alzheimer病，Pick病のグループのほかに亜急性海綿状脳萎縮症，Creutzfeld-Jacob病を中心とする変性疾患グループがあり，疾患相互のカテゴリーは不鮮明で臨床症状，病理組織学的所見も個々の症例によつて多少とも異なつている。老年痴呆，Alzheimer病Pick病では神経組織の経年病変とAlzheimer原線維病変，老人斑の出現が組織病理の基調をなしているが，亜急性海綿状脳萎縮症とその近縁疾患ではこれらの所見を欠くことがむしろ特徴の一つと考えられている。ここに報告する例は臨床的に病初，老人性痴呆と診断されたが，急速に進行する意識障害と特微あるEEG所見から亜急性海綿状脳萎縮症が疑われた症例で，剖検検索すると脳には亜急性海綿状脳萎縮症に特徴的な基本組織病変が認められたが，同時に大脳皮質に神経細胞のAlzheimer原線維変化，老人斑の形成があり，また脳表小動脈にはarteriolar sclerosisの所見が認められている。

Histopathologic findings of a case of subacute spongiform encephalopathy are described in a 63 year old man. The clinical features were marked by progressive mental deterioration with visual failure, speech disturbances, muscular rigidity and myoclonic jerks. The EEG showed generalized loss of a activities with irregular intermission of sharp and slow wave complexes. The death occured 4 months after the start of the first symptoms. Autopsy disclosed marked atrophy of the grey matter extending from the frontal pole to the occipital.
