Following occlusion of the left middle cerebral artery (MCA) in the rat, we reported a marked reduction in cerebral blood flow (CBF) in the territory of the occluded artery and also an absolute increase in CBF in the ipsilateral globus pallidus and substantia nigra (SN). Later, we measured the local CBF and the local cerebral glucose utilization (CGU) using an autoradiographic technique at the chronic stage after MCA occlusion. A similar pattern of increase in CBF was found in the ipsilateral SN. In the measurement of CGU, a marked increase in glucose use was observed in the SN ipsilateral until 2 weeks after the left MCA occlusion. In this model, the SN lies outside the ischemic area, but has transsynaptic connection with the caudate nucleus, which is included in the ischemic focus. The caudate nucleus contains high concentration of an inhibitory neurotransmitter GABA which plays an important functional role in the striato-nigral pathway. Therefore, our hypothesis is as follows : The hyperemia and hypermetabolim in this remote area may be explained as a reflection of altered neuronal function, which is caused by diminished inhibitory output from the ischemic region.
