現在までに日本で承認されている多発性硬化症に対する疾患修飾薬は8種に至り，治療選択の幅は広がっている。その一方で，これらの薬剤を用いても再発を抑制できない多発性硬化症もいまだ経験する。本論では，標準的治療で再発が抑制できない多発性硬化症に対する診断・治療について，自験例を踏まえて概説する。

Abstract

In Japan, disease-modifying drugs for multiple sclerosis have amounted to eight, to date, providing us with various therapeutic choices. However, we still encounter difficult cases resistant to these drugs. This review summarizes the diagnostic and therapeutic strategies for refractory multiple sclerosis by presenting suggestive cases.
