緩下剤長期連用患者に発生した酸性尿酸アンモニウム結石

A CASE OF AMMONIUM ACID URATE URINARY STONE ASSOCIATED WITH CHRONIC ABUSE OF LAXATIVES

　症例は26歳，女性。小児期より重度の便秘症で，緩下剤の長期大量連用歴がある。再発性の左尿管結石に対してESWLを2回施行した。血清電解質，尿酸値には異常を認めなかったが，尿分析にて尿量低下と極度の低ナトリウム尿を認めた。結石分析では酸性尿酸アンモニウムが主体であった。本症例の結石はESWLによって治療可能であったが，このような結石の再発予防には緩下剤の減量が必要不可欠であり，他科との連携のもとに総合的な管理を行うことが重要であると考えられた。

A 26-year-old female who had a history of chronic abuse of bisacodyl visited our hospital because of left flank pain. Left ureteral stones were treated by two series of extracorporeal shock wave lithotripsy. Passed stones were consisted of ammonium acid urate.

Laxative abuse leads to chronic depletion of water and electrolytes. Ammonia production in the kidneys is promoted by chronic potassium depletion, which results in relative supersaturation of ammonium urate in urine. To avoid the abuse of laxatives seems to be essential for the management of these stones.
