急性腎盂腎炎に続発した急性髄膜炎

MENINGITIS AFTER CHEMOTHERAPY FOR ACUTE PYELONEPHRITIS: A CASE REPORT

　72歳，女性が左腰背部痛，発熱にて当科を初診した。左腎盂腎炎の診断でcefotiamを投与しいったん解熱したが，再び熱発し，軽い意識障害や嘔吐もみられた。髄膜炎と考え第8病日から大量のpenicillin Gを持続点滴静注すると劇的に解熱した。腎盂腎炎だけで起こりうる症状であっても，髄膜炎を念頭におくこと，感染症の治療には抗生剤の局所への移行性を考慮することの重要性を痛感した。

A 72-year-old female presented with left frank pain and high fever. A tentative diagnosis was acute pyelone-phritis based on physical findings, pyuria and normal excretory urography. Although drip infusion of cefotiam 1 g, twice a day for 5 days, improved temperature, she became febrile again accompanied with vomiting and slight disturbance of consciousness.
