根治的前立腺全摘除術後の尿道カテーテル抜去困難例

Difficulty in removal of urethral Foley catheter in a patient after radical prostatectomy

　症例は59歳，男性。前立腺癌臨床病期B1の診断で根治的前立腺摘除術を施行した。3-0吸収糸6針にて膀胱尿道吻合を行い，20F尿道カテーテルの先端を切断し留置した。術後14日目にカテーテルの抜去を試みるも，カテーテル先端が膀胱壁に固定されており抜去不能であった。術後21日目にスタイレットを用いカテーテルを抜去した。カテーテルには損傷や結石の付着はみられず，膀胱鏡では前壁に陥凹した，発赤した粘膜が認められた。

An extremely rare case of the urethral Foley catheter trouble after radical prostatectomy is presented. At the operation, a 20F closed Foley catheter was indwelled through the urethra after cutting out the tip of the cathe-ter to make the catheter open ended. Although the catheter could be deflated and was not fixed at the vesi-courethral anastomotic site, it could not be removed. Because of the fixation of the catheter tip, we used sty-let technique under fluoroscopy, and then the catheter was extracted.
