ホルミウムレーザーを用いた前立腺肥大症の日帰り手術

Holmium Laser Resection of the Prostate (HoLRP) in the Day Surgery

　前立腺肥大症に対する日帰り手術としてホルミウムレーザーを用いた経尿道的前立腺切除術を施行した。対象症例は25例，平均年齢は66歳，平均前立腺切除量は6gであった。灌流液には生理食塩液を用いたためTUR症候群は起こり得ず，術中・術後の出血もほとんどみられなかった。治療効果は術後早期よりみられ，IPSSの平均値は術前18.2，術後1か月は7.0，最大尿流率の平均値は術前9.2ml／秒，術後1か月16.2ml／秒といずれも有意に改善し，3か月後も持続していた。ホルミウムレーザーの特徴を生かした本術式は治療効果はもとより，安全性と医療費削減の面からも優れており，日帰り手術に最適な方法であると考えられた。

Transurethral resection of the prostate using Holmium laser was performed in the day surgery setting. A total of 25 patients of a mean age of 66 years old with symptomatic benign prostate hypertrophy in whom serum prostate specific antigen was within normal limit. A 26 Fr continu-ous-flow resectscope was used for the procedure. Mean operating time was 42 minutes, and resected tissue was 6g in an average. Throughout the procedure 0.9% saline solution was used as irrigation fluid, therefore TUR syn-drome could not be occurred. Minimal hematuria was noticed during the procedure and the first 24 hours.
