尿道再発・陰茎転移により持続勃起症を来した膀胱癌

A Case Report of the Priapism in a Patient with Urethral Recurrence and Metastatic Penile Tumor after Total Cystectomy for Bladder Carcinoma

　症例は67歳，男性。膀胱腫瘍にて膀胱全摘除術を施行した（尿道切除術は非施行）。術後11か月頃より，外尿道口からの出血ならびに陰茎部痛が出現した。生検にて尿道再発および陰茎転移と診断した。陰茎は持続勃起状態で疼痛を伴い，様々な鎮痛治療も全て無効であった。その後，疼痛除去を目的に陰茎全摘除術を施行し，術後疼痛は速やかに消失した。激しい局所症状を伴う場合，対症療法としての手術療法は局所コントロールのためによい方法であった。

A 67-year-old man who had undergone total cystectomy for bladder carcinoma without urethrectomy 11 months before, presented priapism and urethral bleeding. Priapism was associated with sever penile pain, for which administra-tion of morphin and epidural block was not successful. Then we performed total penectomy to relieve sever penile pain and the patient became free of sever local symptoms.
