尿失禁根治術Raz法の経験

Raz's Suspension for Urinary Incontinence

　8例の女性腹圧性尿失禁患者に対してRaz法による尿失禁根治術を施行し，全例に良好な短期的尿禁制を得た。当初は"Campbell’s Urology"の記載に従って開始したが，現在では牽引糸をもっぱら腔前壁逆U字型切開内側の恥骨頸部筋膜のみにかけている。これのみでも非常に強靭な抗張力があり，同時にスリング手術と同様の効果を得ることが可能であり，タイプ3尿失禁例1例でも手術は成功した。この手技の変遷に伴って術後残尿期間の延長が認められたため，当初は内視鏡ガイドのみで決定していた吊り上げ強度を膣内診を併用して意識的に緩めることで術後残尿期間の短縮が可能であった。2例は膀胱瘤を合併しており，四点支持法を施行した。

We report our experience of Raz's suspension for eight patients with urinary incontinence. In earlier cases, we performed the operation according to the description in Campbell's Urology, sixth edition. In recent cases, we placed the sutures exclusively on pubocervical fascia, which is located medial to inverted-U-shaped vaginal wall inci-sion. This suture provides a strong anchor, producing the same effect as sling operation. The operation was success-ful even in with type 3 incontinence patient.
