巨大精液瘤の1例

Big Spermatocele:A Case Report

　65歳，男性。左陰嚢内容の腫大に気づき，増大するため当科外来を受診した。穿刺にて精子を含む内容液を320ml吸引し精液瘤と診断した。2か月後と6か月後にも再度増大し，穿刺吸引したため当科に入院し手術を施行した。腫瘤は精巣上体頭部に付着しており，腫瘤のみを切離し摘出した。病理組織学的検査では腫瘤壁は1層の立方上皮から成り立っていた。自験例は本邦第2番目の巨大精液瘤に該当した。

A 65-year-old man presented with left scrotal swelling. Yellowish clear fluid containing sperms was obtained by aspiration, and the fluid volume was 320 ml. A spermato-cele was found near the head of the epididymis, then was surgically removed. Histologically, the wall of spermatocele was composed of epithelial lining of cuboidal cells. This case is the second largest spermatocele reported in the Japanese literatures.
