導尿不可能であった膀胱瘤

A CYSTOCELE CAUSING ACUTE URINARY RETENTION AND NOT ALLOWING URETHRAL CATHETERIZATION

　68歳，女性．急性尿閉で来院．腟口より脱出する腫瘤を認め，用手還納できず，導尿も不可能であった．経腟的に膀胱穿刺を行った後，尿道からカテーテルを留置した．膀胱瘤の診断にて，前腟壁縫縮術を施行した．術後排尿困難は消失した．導尿できない高度な膀胱瘤は極めて稀である．

A 68-year-old woman developed acute urinary retention. A cystocele as large as a fist was observed outside the vagina. Manual reposition of the cystocele and urethral catheterization were impossible. A bladder puncture through the cystocele yielded 1000 ml of urine, and this enabled us to place a urethral catheter. She underwent an anterior colporrhaphy. Postoperatively she was able to void normally. A cystocele that does not allow urethral catheterization has not been reported.
