手術待機中に精索捻転を合併した鼠径部停留精巣の2例

Torsion of undescended testis in the inguinal canal : Report of two cases

　手術待機中に精索捻転を合併した鼠径部停留精巣の2例を経験した．症例1は8か月の乳児．発症8日後の受診時，鼠径部精巣は腫大していたが圧痛はなかった．よって，生後10か月に待機手術を行ったが，左精巣は萎縮しており摘除した．症例2は6歳．発症18時間後に行った緊急手術により右精巣を温存できた．停留精巣患児の家族には2歳までに精巣固定術を受けることと，まれながら手術待機中に精索捻転を来す可能性があることを十分に説明しておくことが本症の早期診断，治療ひいては精巣救済率の向上につながる．

Abstract

We report two cases of torsion of undescended testis in the inguinal canal. Case 1 : A 8-month-old boy was admitted to our hospital with the left groin tender mass lasting eight days. Two months later, elective orchiectomy was performed because of the severe testicular atrophy. Case 2 : A 6-year-old boy visited our hospital due to the right groin pain. Emergency orchiopexy was done 18 hours after the onset of symptom. To obtain an earlier diagnosis and treatment of the twisted cryptorchid testis, an education for both parents and primary care physician is essential (Rinsho Hinyokika 70 : 463-466, 2016).
