巨大脾腫による内側への腎偏位の1例

A CASE OF LEFT RENAL DISPLACEMENT DUE TO SPLENOMEGALY ASSOCIATED WITH PRIMARY MYELOFIBROSIS

　症例は78歳男性で，原発性骨髄線維症にて外来通院していたが，著明な蛋白尿および腹水を認めたため入院した。排泄性腎盂撮影にて左側の無排泄腎の所見を呈し，後日施行した点滴静注性腎盂撮影の第2斜位像において，右腎の前下方に造影剤排泄の遅延を伴う左腎の描出を認めた。CTスキャン，血管造影などによる精査の後，巨大脾腫により左腎が圧排されて右下方に偏位したことが判明した。

A seventy-eight year old male patient was admitted because of massive proteinuria and ascites. Physi-cal examination demonstrated a splenic enlargement extending from the left costal margin toward the umbilicus. Bone marrow and peripheral blood studies were diagnostic of primary myelofibrosis. CT scanning and angiography disclosed the left kidney to be displaced across the midline to the right side by the splenomegaly.
