前立腺肥大症に対する交感神経α1受容体遮断剤の効果

CLINICAL EFFECT OF ALPHA1-ADRENERGIC BLOCKER (PRAZOSIN HYDROCHLORIDE) ON MICTURITION DISTURBANCE OF BENIGN PROSTATIC HYPERTROPHY

緒言

　前立腺閉塞症，神経因性膀胱による排尿障害に対する保存的治療として，近位部尿道の神経支配が交感神経α受容体優位であることが明らかになりつつあり，交感神経α受容体遮断剤（αブロッカー）の投与は，対症的治療だが速効性の効果が予測される。αブロッカーであるPhenoxybenzamine（POB）の有用性はCaineら^1〜3）に代表される多くの報告がある。しかし，POBは副作用や発癌性の問題からわが国では製造中止となつている。高血圧治療薬として最近開発された塩酸プラゾシン（ミニプレス^R）は，交感神経α₁受容体遮断剤であり，その副作用として尿失禁が報告されている^4,5）。われわれは，塩酸プラゾシンを前立腺肥大症による排尿障害に対して投与し，若干の知見を得たので報告する。

The effect of alpha₁-adrenergic blocker (Prazosin Hydrochloride) was studied in 16 patients with benign prostatic hypertrophy. Prazosin, 1-2 mg daily, was administrated orally. Improvements of subjec-tive symptoms were observed in 68.8% of those patiens. There were no significant changes in voided urine volume, but significant improvements were found statistically in residual urine volume, maximum flow rate and average flow rate. In UPP study effect of alpha₁-adrenergic blocker (Prazosin) was the reduction of the pressure at bladder neck and midprostatic urethra.
